IN-HOME SERVICES: Intervention & Assistance, Friendly Visiting, Security Check, Medical Escort, HomeHelp & Respite Care Eligibility: Persons who are 55 year of age and older, persons with physical disabilities who are over 18 and who live in the area bounded by St. Clair Avenue, Bathurst Street, Lake Ontario and Yonge Street are eligible for in-home services following an assessment in their home. |

ADULT DAY SERVICE: Carlton Centre Eligibility: Persons who are 55 year of age and older, persons with physical disabilities who are over 18 and who live in the area bounded by St. Clair Avenue, Bathurst Street, Lake Ontario and the Don River are eligible, following assessment. Referral: Self, Family, Physicians, Discharge Planners, Social Workers, Public Health Professionals. Availability: Monday through Friday BRANCH PROGRAMS: Centres for Seniors- Social, Recreational and Educational Programs Member or Branch Locations and Congregate Dining Eligibility: Persons who are 55 year of age and older, no geographic boundaries.
