COMMUNITY & FACILITY BASED SERVICES & PROGRAMS |
ADULT DAY PROGRAM / FEE FOR SERVICE A facility-based support program that provides a variety of activities in a group setting five days per week. This all-day program, set in a cozy Victorian home in the heart of Cabbagetown allows individuals to make new friends, participate in physical and social activities and partake in a hot and nutritious lunch. The program also provides caregiver respite and support, as well as a transportation service for individuals who do not qualify for WheelTrans. CONGREGATE DINING / FEE FOR SERVICE Facility Based Program - Hot nutritious lunches are served in a group setting. Social and recreational programs are provided. INTERVENTION AND ASSISTANCE / NO FEE FOR THIS SERVICE Provides intervention and assistance to vulnerable and at-risk seniors and persons with physical disabilities and/or their caregivers. The goal of this service is to provide help in critical situations, stabilize and design a plan for the future. FRIENDLY VISITING / NO FEE FOR THIS SERVICE Arranges for a trained volunteer to visit regularly with a senior or physically disabled adult in their home. SECURITY CHECK or REASSURANCE SERVICE / NO FEE FOR THIS SERVICE Provides isolated persons with regular daily contact to reassure them that help is available when needed. May be provided by a personal visit, by a telephone call or other means. MEDICAL ESCORT SERVICES / NO FEE FOR THIS SERVICE Provided by a trained volunteer for medical appointments only. At least 48 hours prior notice. RESPITE CARE / FEE FOR SERVICE Provides supervision, light housekeeping and meal preparation for clients in the home while the caregiver "takes a break". HOME HELP / FEE FOR SERVICE Light housekeeping, shopping, laundry, meal preparation. |
